Drug Abuse Treatment - Adolescent Transitional
Care:
Generally, adolescents who complete Residential, Day
Treatment or Intensive Outpatient Care are referred
to The Palm Beach Institute Addiction Counseling Aftercare
Program. No
cost adolescent aftercare recovery is available to our
adolescents for life so long as they remain clean and
sober. Our
Addiction Counseling Aftercare Program is a step between
completing formal treatment and adolescent aftercare
recovery. The Addiction Counseling Aftercare Program involves a case
management approach to adolescent aftercare recovery.
Adolescents work with their primary therapist
to develop a “transitional care plan”.
The transitional care plan usually plans out
weekly activities to engage and maintain the adolescent
aftercare recovery of the client such as attending 12
step meetings on a predetermined frequency.
This may also involve follow up visits for various
forms of outpatient therapy such as family or individual
therapy or visits to maintain medications.
Adolescents in the Addiction Counseling Aftercare
Program will be monitored by a therapist for compliance
with their transitional care plan.
Generally, adolescents who embrace this service
have a higher rate of successful outcomes.
