Transitional Care: 
 

Generally, clients who complete Residential, Day Treatment or Intensive Outpatient Care are referred to The Palm Beach Institute Aftercare Program.  No cost aftercare is available to our clients for life so long as they remain clean and sober.  Transitional Care is a step between completing formal treatment and aftercare.  This service involves a case management approach to aftercare.  Clients work with their primary therapist to develop a “transitional care plan”.  
The transitional care plan usually plans out weekly activities to engage and maintain the recovery of the client such as attending 12 step meetings on a predetermined frequency. 
This may also involve follow up visits for various forms of outpatient therapy such as family or marital therapy or visits to maintain medications.  Clients in transitional care will be monitored by a therapist for compliance with their transitional care plan.  Generally, clients who embrace this service have a higher rate of successful outcomes.
